Declaration and Informed Consent:
I declare that all personal information supplied to the NMU for the purposes of engagement with eNtsa for technical services,
subsidy evaluation and/or training registration, is up-to-date, is not misleading and that it is complete in all respects. I
undertake to immediately advise the NMU of any changes to my personal information should any of these details change. I
furthermore give the NMU permission to process my personal information and company information, as provided in my client
capture form, feedback form or registration documentation, and sourced from DBE, IEB, Home Affairs, or CIPC and
acknowledge that I understand the purpose for which it is required and for which it will be used.
Purpose for Collection Statement:
The Protection of Personal Information Act, 2013 (POPI Act) has been promulgated and implemented on 1 July
2020. All personal identifiable information provided by you shall be treated in accordance with this statute and only used for
engagement, reporting as required by TIA and academic and academic administration purposes, as indicated in the
University’s Privacy Policy. By providing your information, you are giving consent for the use of all your personal identifiable
information, provided to the University, for the aforesaid purposes.
